Abstract: Introduction: Delusional infestation (DI), also known as delusional parasitosis or Ekbom's syndrome, is characterized by a patient's fixed belief that one's skin or other parts of the body or one's immediate environment are infested with small living or inanimate pathogens, against all medical evidence for this. The delirium of infestation can however be secondary to a medical pathology, which should be sought. Case report: A 52 year old morrocan women patient who has had paresthesias in her neck and her limbs for two years. She is convinced that she is infested by a parasite that she has been drinking in a glass of water. The evolution has been marked by an accentuation of the symptomatology and recent appearance in a month of an intermittent dysphagia to the solids. The psychiatric assemssment reveals a monothematic delusional syndrome systematized with total adhesion. The patient improved on atypical antipsychotic after eight week of treatment. Conclusion: This case of Ekbom syndrome demonstrates the importance of early management of pruritus or chronic burns and of the search for an underlying organic pathology, a delirium of infestation representing a potential complication. The most important problem is the clinical management of patients because they are often reluctant to engage in a meaningful therapeutic relationship.

Abstract: Introduction: Melancholia has disappeared from the current classification manuals and is reduced in DSMV to the depressive disorder characterized by melancholic characteristics which, in our opinion, does not deeply touch the intimate functioning of the patient and his particular clinical structure belonging to the field of psychosis. Objective: Through two clinical cases, the first one of a melancholic patient and the second one of a paranoid patient, we will approach the peculiarities of the delirium associated with each of these clinical entities through a fine analysis of the delirious construction and organization. Cases report: Our melancholic patient presents a maladaptive guilt, delusional, says he gives off a bad smell, he is persecuted by others who are ashamed to meet him, it is because he is a waste that others hate him and it is because he feels bad that they make the mistake of knowing him, in fact our patient was persecuted by the entourage which according to him makes the mistake of knowing him despite he is the culprit, the great eternal culprit. Our paranoid patient had a delusion of persecution by projection onto others, a megalomaniac persecution which, together with her high contact, made the bed of a false and hot diagnosis of a hypomanic episode. For our patient the entourage is unworthy and source of her misfortune. Conclusion: What paranoia specifically reaches by way of projection, melancholia achieves by way of introjection.

Abstract: Youth drug abuse is a high-profile public health concern worldwide. Besides this drug abuse is one of the major issues in Tanzania. The youth in Tanzania are often involved in trafficking and consuming illegal drugs like cannabis. Illegal substance use among students can negatively impact their health in total, could negatively impact their educational grades and performances and also can give rise to maladaptive behaviors and personality traits. Studies in this area are scanty in Tanzania. This gap in knowledge prompted the design of the current study. The present study surveyed the status and prevalence of substance use among the students of Tabora polytechnic college of its Tuli campus; where 350 students of age group 15-30 years from different departments were being interviewed using ADA baseline questionnaire. Substances which are commonly abused by the students are found to be Alcohol (31.43%), Cigarettes (30.86%) and Bhang (19.71%). The Believe that alcohol and drug use facilitates sexual opportunities (32.5%) and facilitates connection with peers (31.7%) has escalated the use of drugs and alcohol and has been revealed that most of the students abuse drugs during weekends (23.71%) and after evening lectures (18.34%). The major sources of drugs and substances of use are from among the students themselves (39.72%).

Abstract: Economic evaluation is becoming an increasingly important part of public health decision- making. It is useful for rationalising the allocation of available resources in order to maintain a quality health care system that is accessible to all. The economic costs of mental illness are a barrier to access to quality psychiatric care. This study aims to determine the average annual cost of mental disorders in public psychiatric hospitals in Côte d'Ivoire. This descriptive cross- sectional study focused on 100 patients over 15 years of age with psychiatric pathologies who had at least one psychiatric hospitalisation in the two psychiatric hospitals of the country, namely Bouaké and Bingerville. The study showed that the caretakers were parents (77%) with an average monthly income of 185,000 CFA francs and were working in the informal sector (43%). The most frequent diagnosis was acute psychotic disorders and related disorders (51%) and antipsychotics were the most commonly used, including the combination of Haloperidol and Chlorpromazine. The average annual cost per patient was estimated at 303,132.77 FCFA and 523,059.88 FCFA respectively in Bouaké and Bingerville. It is essential that policies look at subsidising mental health care in order to alleviate the economic burden of people living with mental illness.
