Abstract: Puerperal psychosis is an acute psychiatric disorder with a predilection for women, one pregnancy in 1000. In the past, it was never mentioned in men. This disorder can be recurrent (PPR) under certain conditions and its manifestations are variable. The objective of this clinical case report is to bring to the attention of the scientific world, among the rare cases reported, the recurrent paternal puerperal psychosis that occurred in a 33-year-old male subject in our neuropsychiatric hospital of the Brot hers of Charity Saint Vincent de Paul of GOMA/NK in the DRC. The clinic had highlighted during the psychiatric observation, a delirious syndrome, an anxious agitation, a total and mixed insomnia with a personal and family psychiatric history. The questioning invading the paternal ego weakens him in front of a questioning due to the new responsibilities that some have difficulty to manage, of only one neck it can upset the concerned one what would bring it to a decompensation towards a paternal puerperal psychosis. Paternal psychosis, whether recurrent or not, is therefore a real emerging pathology that requires special attention, contrary to the old theories that referred to this entity only to women in postpartum.

Abstract: Adolescent mental health problems are a public health challenge around the world and particularly in a country like Sierra Leone that has experienced war and the Ebola virus epidemic. Adolescents may suffer from psychological distress with school and family determinants. The goal of this study was to determine the prevalence and factors associated with psychological distress among school-going adolescent in Sierra Leone. This involved the secondary analysis of the data from the Sierra Leone, 2017 Global School-based Student Health Survey (GSHS). The dependent variable was “psychological distress,” which was assessed using two questions: 1) “In the past 12 months, how often have you worried about something to the point of not being able to sleep at night?” and 2) “In the past 12 months, how often have you felt lonely?” A weighted analysis of the data was performed using the software STATA version 12.0 (Stata Corporation, College Station, Texas, United States). One in two adolescents surveyed was male (51.53%). The average age of these school-going adolescents was 15.32±1.73 years. The average age of the boys was 15.37±1.74 years and that of girls was 15.15±1.66 years. The prevalence of psychological distress in this population was 26.81% CI95%=[24.91 - 28.70]. The boys seemed to suffer more of psychological distress than the girls (28.95% CI95%=[26.12 - 31.78] vs 24.54% CI95%=[22.05 - 27.01]). The factors associated with psychological distress among school-going adolescents were bullying, lack of respect for their privacy, alcohol and/or cannabis use, multiple sexual partnerships and sedentary lifestyle. The psychological distress among adolescents in Sierra Leone is real. Knowing the risk factors for psychological distress in adolescents will improve its prevention and promote the mental health of these future adults.

Abstract: Equine-Assisted Therapy (EAT) is a complementary therapeutic approach for children and adolescents with post-traumatic stress disorder (PTSD), and recent scientific studies show the benefits. Therapy with the mediation of the horse facilitates the therapeutic alliance and improves social skills, emotional regulation, and resilience. The purpose of this study is to describe goals, modalities, and benefits of equine-assisted therapy for adolescents with PTSD, and to analyze how and why these results are obtained. Ten EAT sessions were offered to two adolescents with PTSD following a terrorist attack. According to the observed symptoms, the therapeutic objectives and modalities are established and described. The evaluation is based on a self-evaluation by the adolescents, and on an EAT observation grid. The results show a positive evolution, with a very strong alliance and participation. Equine-assisted therapy facilitated adherence to care, improved socio-emotional and cognitive functioning, helped regulation in behavior and emotions, restored confidence and self esteem, and provided wellness. Understanding the action of EAT for children and adolescent with posttraumatic stress disorder helps to explain the benefits and effectiveness of this complementary therapy. The factors involved are analyzed according to several approaches: psycho-affective, cognitive, social, neurobiological, and therapeutic. Studies need to be continued with larger samples to obtain evidence-based data and to validate the results of equine-assisted therapy in adolescents with PTSD.

Abstract: Some manifestations attributed to a primarily psychiatric disorder may result from an identified or unidentified neurological lesion; in other words, psychiatric symptoms are often a common determinant for both neurological pathologies and psychopathological disorders. The objective of this work was to study the radio-clinical profile of psychiatric manifestations attributable to a neurological process. We therefore carried out a retrospective cross-sectional study with a descriptive aim. The survey took place from December 2018 to March 2019 at the Abidjan Addictology and Mental Hygiene Service (SAHM) of the National Institute of Public Health (INSP). The period studied was 08 years (2012-2019). Thus, 47169 patient files were identified over this period (2012 to 2019). A total of 125 patient files meeting the inclusion criteria were selected from an exhaustive sampling. It should be noted that the use of specific biomarkers was not possible in our context. There was a modest female predominance of 52.80%, a mean age of 65 years. Clinical characteristics revealed a history of hypertension (23.2%). Call signs were amnesia (80.5%), incoherent speech (45.5%) and the delirium-hallucination dyad (40.9%). A dementia syndrome was evoked in a proportion of (61.6%); it required neuroimaging. In 96.8% of the cases, the brain scan revealed cortico-subcortical atrophy lesions (66.4%), but MRI could only be performed in (3.2%). Degenerative, vascular or mixed etiologies were evoked in 34.4%, 15.2% and 30.4% respectively. The risk of excessive psychiatry and misdiagnosis of behavioral disorders reveals the interest of a more elaborate neuropsychiatric evaluation and the use of neuroimaging.

From Platonic Love to Mimic Love: The Psychic Implications of a Thousand-Year-Old Homophobia

Abstract: In Ancient Greece, Platonic love referred to a sexual, intellectual, and emotional relationship between two men, one young and one older. It was the most valued relationship by the Greeks, but it was censured by Christians. They redefined platonic love as a friendly relationship because they have made heterosexuality the exclusive basis of their society. Their intention was to produce as many humans as possible in order to colonize other populations. Later, secular democracies have resumed their purpose by repressing homosexuals ever more violently. Their extermination caused no judgment, no compensation, for no country. Still today, homosexuals represent internationally what men should not be and are usually the whipping boy. Although a great part of the culture was made by them, no society values them. On the contrary, all of them seem to censor the sexual inclinations of their great men. Homosexuality is rather presented as a marginality, a sin, a disease, or a defect that should be avoided. It can generate aggression in those who perceive it, and it causes fear, shame, or guilt in those who experience it. All these feelings that gays internalize, lead them to submit to the only social model that exists in the West: heteronormativity. Moreover, censorship let them think that there is no other social norm than reproduction. So instead of questioning it and creating homonormativities as it existed before the Christian era, they identify with heterosexuals and mimic their way of life. They also want to live together, to start a family, to marry, to be faithful, to have children by assisting reproduction/surrogacy, or by adopting if the heterosexuals of their country allowed them to do so. From the platonic love that valued homosexuality to the mimic love that denies it, gays of the West are therefore driven to imitate heterosexuals, without being aware of the psychic process that lead them to it. Some of them break free and invent their own way of life by doing psychotherapy with a professional who is himself aware of the situation, but outside of this case, most of gays remain locked in a heteronormative model that oppresses them. Would recognition of censorship and crimes they have been victims of for millennia allow them to emancipate from the identification with their aggressors and from the heteronormativity?

Abstract: The purpose of this study is to describe potential precipitants of re-hospitalization and dis-engagement during the first three years of diagnosing a psychotic episode, in Abu Dhabi, United Arab Emirates (UAE). A retrospective cohort study that included 56 patients, with the first episode of psychosis (FEP). 55.4% of the sample were men, 52% were single, 80% were of Arab ethnicity, and 50% were unemployed. The mean age was 29.51 years old. 42% had non-affective psychosis. During the first three years of psychosis, more than three-quarters of FEP patients were re-hospitalized. The rate of service disengagement was 64.8%. Bipolar affective disorder and schizophrenia patients had a higher re-hospitalization rate compared to patients with other diagnoses (p<0.001). Re-hospitalization was significantly higher for males and single patients (p=0.047). There was no significant correlation between re-hospitalization and the length of hospital stay, with the period of untreated psychosis, or with age at onset. Being single was the only significant correlate with service dis-engagement (p<0.001). Single status (p=0.038) and Non-smoking (p=0.008) were the only significant correlate with re-engagement. We concluded that re-hospitalization and dis-engagement rates were high in Abu Dhabi, and the predictors were unmodifiable factors such as gender, marital status, and psychiatric diagnoses. The findings of this study could be used to develop future services, such as establishing dedicated assertive intervention programs for patients after their first episode of psychosis to improve treatment outcomes.

Abstract: Introduction: Cyberaddiction can impact all aspects of people's lives, especially adolescents and young adults. Knowledge about the prevalence and related factors of these phenomena is therefore needed for prevention and treatment planning. Objective: To assess the prevalence and factors associated with cyberaddiction among Ivorian students. Material and methods: This was a descriptive and analytical cross-sectional study that involved 388 students from Alassane Ouattara University in Bouaké over a period of 03 months (June - August 2017). The students were assessed using Young's Internet Addiction Test (IAT). Results: The prevalence of cyberaddiction was 11.08%. The associated factors were femalesex (p=0.010), usage modalities such as daily connection time greater than 8 hours (p<0.0001) and less than 1 hour (p<0.0001), network usage (p=0.009), night internet use (p=0.0047). The consequences on life were sleep disturbances (p=0.009), homework equality (p<0.0001), decreased performance (p<0.0001) andisolationendance (p<0.006). Conclusion: These results show the need to raise awareness among the population, especially students, about the harmful effects that excessive use of the Internet can have on their lives.
